Spectrum of Cervical Lesions in Pap Smear with Histopathological Concordance: A Cross-sectional Study

Introduction: Cervical cancer is amongst the leading causes of death in females. According to World Cancer Statistics, global incidence cervical 6.5% and mortality related it 7.5%. The most effective way screen thus treat early stages achieved by screening patients with Papanicolaou (Pap) smear. Aim: To assess spectrum lesions Pap smears, classify them as per Bethesda System reporting Cytology compare cytology results histopathology where available. Materials Methods: present study was a cross-sectional carried out for period two months from August 2022 September Rohilkhand Medical College Hospital, Bareilly, Uttar Pradesh, India. slides smear were reported 2014 cytology. Comparison histopathological findings done cases whenever biopsy or hysterectomy specimen received. Data collected, entered compiled Microsoft excel followed analysis using software Statistical Package Social Sciences (SPSS) 23.0. data represented frequency validity calculated terms sensitivity, specificity, Positive Predictive Value (PPV), Negative (NPV) accuracy. p-value Chi-square test significance set at p<0.05. Results: All received during duration included which 400 consecutive PAP smears. Epithelial cell abnormality seen 17 (4.25%) cases. Atypical Squamous Cells Undetermined Significance (ASCUS) 8 (2%) cases, Low-grade Intraepithelial Lesion (LSIL) 2 (0.5%) High-grade (HSIL) 1 (0.25%) case, Cells- cannot exclude high-grade squamous intraepithelial lesion (ASC-H) 4 (1%) Cell Carcinoma (SQCC) Concordance histopathathology 33 37 overall PPV, NPV diagnostic accuracy 90%, 88.9%, 75%, 96% 89%, respectively. 0.00001. Conclusion: Cases diagnosed on low-grade epithelial including ASCUS LSIL should be kept follow-up, whereas ASC-H, HSIL SQCC, appropriate treatment planned.
